We understand that as a new patient, you may have questions before your visit. Don’t hesitate to contact us if you have questions that aren’t answered below.

Scheduling an appointment
When scheduling an appointment, please be prepared to provide your insurance information and the purpose of your appointment so we may better prepare for your visit. If you require us to complete DMV forms, please mention this as it usually requires a separate visit.

What should I bring with me?
On your first visit, be prepared to provide us with your medical history and any allergies or pre-existing
conditions. Please also bring your current medications including eyedrops, eyeglasses and contact lenses including your prescription. If your insurance plan requires a referral to see specialists, please bring an authorization from your primary care doctor or health insurance plan.

Northern California Cornea Associates accepts most major insurance plans but does NOT accept the following plans. Patients with these plans will be held responsible for payment at the time of service.

Where are surgical procedures performed?
Simple procedures can be performed in our office, however the majority are performed in outpatient
surgical centers. In Walnut Creek, we partner with Aspen Surgery Center. In Oakland, we work with
Eye MD Laser and Surgery Center and Alta Bates Summit Hospital. All sites provide cutting-edge
technology and friendly, efficient staff.
